1. You have booked an appointment with us
2. You have filled in our online enquiry page

1. Your referring veterinarian for relevant medical history
2. We use the information that you have given us in order to contact you regarding appointments & results, and to establish relevant patient assessment
3. We may share this information with your referring veterinarian or insurance company where required.
